Everyone was trained. The deck went out. Attendance was ninety-something percent. The box is checked.&lt;/p>
&lt;p>Then something happens on a Tuesday that the training was supposed to prevent, and the same room asks how, because everyone knew the procedure.&lt;/p>
&lt;p>They did not, though. Or rather, the word &amp;ldquo;knew&amp;rdquo; was doing far more work than anyone noticed. It was standing in for at least four completely different states, and the gap between them is where competence quietly rots.&lt;/p></description></item></channel></rss>
